R5125.6 ST. LOUIS BOARD OF EDUCATION REGULATION STUDENTS ELEMENTARY, MIDDLE AND SECONDARY Progress Student Records -- Record of Requests and Disclosures 1. A record of each request for disclosure, including requests for disclosure that were declined, of personally identifiable information from the student records will be maintained and kept as a part of each student's record, with the exception of disclosures to authorized school personnel and disclosures of directory information. 2. The record of requests for disclosures and disclosures shall contain the following minimum information. a. The names and addresses of parties who have requested or obtained personally identifiable information from the education records of the student. b. The records reviewed. c. The legitimate interests these parties have in requesting or obtaining the information. d. The date the request for disclosure was made and/or given to the requesting party. 3. The record of requests and disclosures may be inspected by the parent of a student or the eligible student, by the superintendent of schools or designee who are responsible for the custody of records and by state and federal officials in connection with the enforcement of federal legal requirements for student record keeping, and/or audit and evaluation purposes.
